Abstract

Perifollicular inflammation is a key factor in several hair loss conditions, yet its objective assessment remains a challenge due to the lack of standardized measurement tools. The Salinger Scale, developed by trichologists in collaboration with the International Association of Trichologists (IAT), aims to provide a non-invasive, clinically reliable method for evaluating perifollicular inflammation severity.

The scale consists of a five-point classification system ranging from 0 (no inflammation) to 4 (extreme inflammation), using high-resolution trichoscopic images. Over 2,000 de-identified images from clinical settings in Sydney and Nairobi were analyzed, classified, and reviewed to ensure accuracy and usability. Unlike traditional invasive methods such as biopsies, the Salinger Scale allows for objective, reproducible, and patient-friendly assessment.

This standardized tool has the potential to enhance clinical decision-making, improve treatment evaluation, and facilitate global research collaboration in trichology and dermatology. Future studies will further validate and refine the scale based on expert feedback, ensuring its integration into everyday clinical practice.
